    Plaquenil retinopathy pathogenesis

One such commonly used medication for dermatologic and rheumatologic inflammatory conditions is hydroxychloroquine (Plaquenil), a chloroquine derivative. It is used to treat many diseases including malaria, rheumatoid arthritis and systemic lupus erythematosus.

Apr 28, 2009 The ocular pathology of autoimmune retinopathy is demonstrated in a 62-year-old female patient with systemic lupus erythematosus SLE who presented with typical clinical autoimmune retinopathy. Macroscopically, there were multiple depigmented lesions. Ocular symptoms of retinopathy include blurred and partial loss of central vision, side vision and in the later stage, night vision. Symptoms of corneal deposits include haloes and glare. Clinical research has resulted in precise screening protocols and safe dosing guidelines to prevent ocular toxicity and detect retinal damage at an early stage. Plaquenil or hydroxychloroquine is under the quinolone family. These medications are initially used in the prophylaxis and treatment of malaria. Plaquenil has replaced chloroquine because it produces less retinal side effects.

It is imperative that patients and physicians are aware of and watch for this drug’s ocular side effects. Retinal toxicity from hydroxychloroquine is rare, but even if the medication is discontinued, vision loss may be irreversible and may continue to progress.

Decades of research into the pathophysiology and management of diabetic retinopathy have revolutionized our understanding of the disease process. Diabetic retinopathy is now more accurately defined as a neurovascular rather than a microvascular disease as neurodegenerative disease precedes and coexists with microvascular changes. Rheumatologists use hydroxychloroquine sulfate Plaquenil, Concordia Pharmaceuticals to treat autoimmune diseases, namely discoid or systemic lupus erythematosus, rheumatoid arthritis, Sjögren syndrome, and malaria. When taken at high doses and for long durations, hydroxychloroquine has been known to cause parafoveal retinal toxicity. Aug 30, 2011 Diagnosis Hydroxychloroquine-induced retinal toxicity Discussion. Chloroquine CQ and hydroxychloroquine Plaquenil HCQ have been used for many years, initially for the treatment of malaria but now more commonly for the treatment of inflammatory diseases such as rheumatoid arthritis and lupus 1. It is now considered for new disease applications in diabetes, heart disease and adjunct cancer therapies 2.

Long-Term Side Effects of Plaquenil for Rheumatoid Arthritis. Aug 14, 2017 Plaquenil can cause serious side effects when used in high doses for chronic diseases such as RA over the long term. The New Zealand Dermatological Society recommends that to decrease the risk of retina damage, no more than 200mg of Plaquenil should be taken a day, and a dose of 400mg per day should be taken no longer than a few weeks.
